For valuable consideration received, I hereby give appie, LLC hereafter referred to as the photographer and/or videographer, the absolute right and permission, with respect to the photographs or video that the have been taken of me or in which I may be included with others:
(a) To copyright the same under appie, LLC office and/or any other trade name that the photographer/videographer may choose.
(b) To use, re-use, publish and re-publish the same in whole or in part, individually or in conjunction with other photographs/video, in any medium and for any purpose whatsoever, including (but not by way of limitation) illustration, promotion and advertising and trade
(c) To use my name in connection there with if the photographer/videographer chooses.
I hereby release and discharge the photographer from any and all claims and demands arising out of or in connection with the use of the photographs, including any and all claims for libel. This authorization and release shall also ensue to the benefit of the legal representatives, licensees and assigns of the photographer as well as, the person(s) who purchase said photographs/video and the person(s) who have hired the photographer/videographer to take said photographs/video. I have read the foregoing and fully understand the contents thereof.
